Duties
PERFORMANCE RESPONSIBILITIES:
- Serve as a liaison between the school and the families of referred students regarding health information and interpretation.
- Participate, as needed, in multidisciplinary diagnostic progress and educational placement staffings for students referred for special education and/or related services.
- Provide, on a consultant basis as needed, medical information and interpretation and serve as a liaison between medical personnel and the school.
- Provide, as needed, medical information and interpretation aimed at alleviation of medical or emotional problems of students.
- Serve as a liaison between the school system and community health and social welfare agencies.
- Maintain medical records on students and supervise the initial administration of medication to students.
- Carry out awareness/identification activities related to service delivery.
- In cooperation with the Assistant Superintendent of Special Services arrange for physical and dental examinations.
- Assist in providing in-service training, workshop, parent education, prepare required reports an research and dissemination activities as needed.
- Prepare informational materials on health regulations.
- Service as a consultant to school personnel in areas of professional expertise.
- Provide for annual vision screening in accordance to accepted standards, techniques and criteria as determined by the State Superintendent of Education and the Director of Public Health.
- Implement schedule for annual vision screening with the principal and screening technicians.
- Maintain records of vision screening for each school.
- Assist in follow-up of students who are referred to a physician and/or vision consultant.
- Interpret the physician’s report to the student’s teacher.
- Provide spot screening test for teacher and psychologist referrals during the school year.
- Provide for annual hearing screening in accordance to the Child Hearing Test Act implemented by the State Superintendent of Education and the Director of Public Health.
- Implement schedule for annual screening with principal and screening technicians.
- Maintain records of hearing screening for each school.
- Assist in the follow-up of students who are referred to physician and/or audiologist.
- Assist in the follow-up of children with permanent hearing loss.
- Interpret the physician’s and/or audiologist’s reports to the student’s teacher.
- Provide for teacher and psychologist (spot) referrals for hearing screening during the school year.
- Prepare forms for recording immunization data from student health records.
- Develop and implement a plan for reviewing student health records.
- Provide principal data on student compliance with Health Department Regulations.
- Inform principal of current requirements for record keeping of student health records.
- Maintain information relative to number of records reviewed and significant variables.
- Perform such other duties and assume such other responsibilities as may be assigned by the Assistant Superintendent for Special Services.
Qualifications
QUALIFICATIONS-
*MUST BE A LICENSED REGISTERED NURSE IN THE STATE OF ILLINOIS
*MUST PASS A PRE-EMPLOYMENT PHYSICAL AND DRUG/ALCOHOL SCREENING
*MUST PASS A CRIMINAL BACKGROUND CHECK
SALARY-
*BASED ON EDUCATION AND EXPERIENCE, PER THE TEACHER COLLECTIVE BARGAINING AGREEMENT
